Primary Care Network is a collective of family physicians and primary care providers practicing in the same region. No commitment is required from members to receive information about what is happening in the primary care landscape.

Decision-making body that represents the PCN at system-level tables. They work together to engage the network to improve healthcare delivery and advocate for system change.

We listen and identify concerns and challenges that primary care providers face by meeting regularly
We represent that at key decision-making tables, ensuring that your voice is heard
We co-develop innovative solutions to improve patient care and increase access to local resources
We update our network on what is happening at the system-level
We organize networking and continuing CME events to build capacity within our primary care provider network
