West Toronto Primary Care Network Registration Form

Please note the information you provide will be kept secure and used only to support your participation in the Primary Care Network. 


Register to be a part of the primary care network and unlock access to a multitude of benefits including:

  • Eligible to participate in OHT/PCN committees, working groups and task groups
  • Eligible to participate in funding opportunities through the OHT/PCN
  • Eligible to receive practice supports through OHT/PCN
  • Added to the PCN Newsletter with exclusive updates and the PCN WhatsApp chat

* Comprehensive Family Medicine Member and Primary Care Nurse Practitioner Member are able to sit on Primary Care Council.

** If you have selected 'Executive leader, non-clinical', please share the name of the organization in the next field.

* If you are interested in attaching patients through a supported intake where the number of patients and frequency are determined by you, please contact the PCN administrative lead (sbraganza@communiticare.org) for more information.
