The West Toronto Primary Care Network (WT PCN) is a collective of family physicians and primary care providers working together to strengthen primary care in West Toronto. We believe that when primary care is empowered and supported, the entire healthcare system improves.

Our Journey 

In 2019, the Ontario government introduced Ontario Health Teams (OHTs) to improve care coordination by integrating healthcare organizations and providers in the same geographic region.

With the creation of West Toronto Ontario Health Team (WT OHT), the WT PCN emerged to strengthen and elevate the voice of primary care in shaping local healthcare delivery. As a network of family physicians, nurse practitioners, and primary care teams, we work to make sure comprehensive primary care is well supported as it is the foundation of health care.

As the WT PCN continues to evolve, amplify primary care provider voices and create more primary care community leaders to help the system, here is the current leadership structure that is helping us grow together 

What We Do

We connect, support, and advocate for primary care providers, ensuring they have the resources, collaboration, and representation needed to improve patient care and make practice easier. 

Our work is guided by the CASA model: 

  • Connect providers and healthcare partners to improve communication and care coordination 

  • Act by implementing practical, co-designed solutions that benefit providers and patients 

  • Support sustainable practice by offering digital tools and leadership opportunities 

  • Advocate for comprehensive primary care as the foundation of a strong healthcare system 


How We Do It

The WT PCN strengthens primary care by promoting collaboration, co-creating innovative care solutions and aligning with system-wide goals. This allows us to advocate effectively for system improvements that benefit both providers and patients.

We do this through a council of primary care providers who understand the needs of West Toronto and the importance of continued advocacy.

Primary Care Council (PCC) is the decision-making body representing primary care providers in West Toronto. The PCC provides strategic direction, oversees engagement initiatives, and represents the voice of the primary care network at regional and provincial decision-making tables. 

Meet Your Advocates

Role of the PCC 

  • We facilitate access to WT OHT initiatives such as SCOPE (Seamless Care Optimizing the Patient Experience), system navigation services, and practice facilitation resources. We also play a role in how funding is allocated within the OHT and advocate for better primary care supports

  • Through newsletters, CME events and peer discussions, we keep primary care providers in the West Toronto region informed and engaged with system-wide developments and hear their perspectives and needs to help make practice easier. 

  • We build partnerships with community agencies, home care providers, hospitals and mental health services, to develop integrated care solutions and remove barriers to access, making it easier for patients to receive the care that they need. 

Decision-Making Approach 

The WT PCN is jointly accountable to the WT OHT Steering Committee and our network members. By aligning primary care efforts with the broader objectives of the WT OHT and Ontario Health, we help create a more effective, responsive, and patient-centered healthcare system.
