The West Toronto Primary Care Network (WT-PCN) is a collective of family physicians and primary care providers working together to strengthen primary care in West Toronto. We believe that when primary care is supported, the entire healthcare system improves.

Our Journey 

In 2019, the Ontario government introduced Ontario Health Teams (OHTs) to improve care coordination by uniting healthcare providers in the same geographic region. 

With the creation of West Toronto Ontario Health Team (WT OHT), the WT-PCN emerged to empower primary care providers to have a stronger voice in shaping local healthcare delivery. As a network of family physicians, nurse practitioners, and primary care providers, we work to remove barriers to improve patient care experience while also ensuring that primary care is at the center of system-wide decision-making. 

WT-OHT expresses support for developing a physician network.
