  • Ontario Primary Care Networks are collectives of local family physicians, nurse practitioners and primary care teams and organizations that are being developed alongside Ontario Health Teams to ensure primary care is the foundation of health system planning. Primary Care Networks work towards understanding the local landscape and needs of primary care practices in its geography to advocate for localized resources. PCNs are accountable to both its members and the OHT and maintains a bidirectional relationship with the OHT with consensus decision making. There is no cost or time commitment to join.

  • It is the leadership body of the PCN which engages and communicates with members and represents the primary care perspective at OHT planning and decision tables. It consists of up to 8 family physician/Nurse practitioners that meet monthly PCC member roles are compensated with a 2-3 year term and will be posted on a rotating basis.     • Strengthens Primary Care's Role in the Health System 
      We ensure primary care has a strong voice in regional health system planning, advocating for it as the foundation of healthcare. 

    • Understands and Supports Local Needs 
      We analyze the local primary care landscape to identify gaps and develop strategies that make it easier to deliver care. 

    • Co-Designs Solutions to Improve Care 
      We collaborate with partners and funders to design innovative approaches that improve patient care and enhance access to services. 

    • Engages and Communicates with Providers 
      We keep our members informed and involved through newsletters, regular updates, and engagement events. 

    • Helps Practices Thrive and Adapt 
      Enable practices to access resources, navigate transitions, and build capacity through coordinated supports.

    • Comprehensive family medicine member,
